Please make sure to write it exactly as desired.

1. DATE LINE (The larger of the two custom lines)

The date will be formatted just like the example in the photo. Months/days with single digits will be up to you as far as formatting is concerned. For example, if your anniversary was on January 1, 2012. You could specify the following:

01.01.12 or 1.1.12

2. NAME LINE (The smaller of the two lines)

Names - This line will be capitalized. Just let us know the names you want on this line. If there are too many characters for one line, we will add one more line and we will use it for the last name.

3. PHONE NUMBER (really important so that we can contact you with questions if necessary)
